Identity theft…how do I find out who did this?

We are currently resolving some very fraudulent claims made against my friend. He had went to apply for a loan after his insurance declined to cover IKE damages and was slow to pay for other major damages. He is an old man with perfect credit…he is after all retired on fixed income.
The credit bureaus recorded Montgomery county had made a lien against his current property almost two decades ago… Its the wrong name and lists aliases. My friend and I have known each other for decades…he’s apart of the family almost and has no criminal background whatsoever! The county says they don’t report to credit bureaus, nor place liens, they just auction the property off for the $$$.
Equifax just list some obscure address for a court that were not even sure exists.
…anybody resolve these matters ..how did you get it fixed?
Why would the VA do that…with out investigating..and was that legal? Can he still get his money back or is that a lost cause…?
We were told its a scam but..upon his sell of the home…money would have been paid to the lien holder.
How do we find out who the lien holder is if the the county has no record of this lien?
He was never served with a court order or anything. Do we need to get a lawyer?…

The government even siphoned money off for a loan years ago…for a VA loan for the property he does not own…and never applied for. He said the VA was adamant and took the money with out investigation!
